Enhancements to Social Determinants of Health

Link Problems to Social Determinants of Health

We’re adding a new option to the patient chart that makes it easier to connect problems/diagnoses to documented social determinants of health (callout 1):

  • New SDOH Checkbox: When adding or editing a problem, you’ll now see an SDOH checkbox.

  • Information Icon: An “i” icon appears next to the checkbox. Hover over it to see the following message: Check to link to SDOH Assessments, Goals, and Interventions.

  • Automatic Association: If selected and saved, the problem will be linked to the 22 determinants available under Patient Hx > Social Determinants of Health.

This enhancement ensures providers can clearly connect a patient’s medical problems with their related social and environmental factors, helping to support more holistic care planning.

Start and target dates for interventions and goals

We’re updating how you document interventions and goals for Social Determinants of Health with the following:

  • New Date Fields: When documenting interventions and goals for any of the 22 determinants, you can now enter a Start Date and a Target Date (callouts 1 and 2).

  • CCDA Integration: The Start Date and Target Date values will also be displayed in the CCDA for continuity of care.

These updates ensure that you can capture both when interventions and goals were first documented and when they’re planned to be acted upon.

Document patient’s disability status

We’re adding a new Disability Status option to the Living Environment section in Social Determinants of Health to help practices better tailor care plans.

This update includes:

  • Disability Score Calculation: Patients or practice staff can complete a short survey to assess disability status. The system will automatically calculate a score based on the responses.

  • CCDA Integration: The disability interpretation, explanation, and total score will be included in the CCDA export, supporting interoperability and standardized data exchange.

Enhancements to cognitive status documentation in patient charts

To better support cognitive assessments for oncology patients, we’re adding the Mini-Cog Cognitive Assessment to iKnowMed. This update allows you to document a patient’s cognitive status and record their total score directly in the chart.

What’s New

A new cognitive status experience is available under Clinical Profile > Observations. The experience includes:

  • A link to the Mini-Cog Assessment (callout 1), which opens in a new tab so you can complete it without leaving the patient’s chart.

  • A new Score (0–5) field (callout 2) where you can document the patient’s total Mini-Cog score.

  • Documented assessments and scores displayed under the Performance Status section (callout 3).

  • All actions, including the total score, tracked in the patient’s Audit History (callout 4).

Clear Value Plus regimen sorting update

We’re improving the way regimens are sorted in Clear Value Plus to enhance decision-making and patient outcomes by placing the regimens with the highest level of evidence in the forefront.

What’s changing

Currently, Value Pathways regimens appear at the top of the regimen table in green, sorted alphabetically, followed by all other regimens, also sorted alphabetically.

After this release, regimens will be sorted by the NCCN Category of Evidence (COE) instead. This means regimens with a COE of 1 will be displayed first, followed by COE of 2A, and so on.

Value Pathways regimens will remain at the top within each category and will continue to be displayed in green.

Within each COE, Value Pathways regimens will be listed alphabetically first, followed by all other regimens in that COE, also sorted alphabetically.

Additional Enhancements (A-Z)

Clinical Profile > Patient Hx

We’re adding a new Start Date field under the Social History > Working Environment and Lifestyle sections of the Patient Hx tab.

The Start Date field under Working Environment allows you to document when a patient first started their job. The Start Date field under Lifestyle will appear next to each exercise activity, allowing you to record when the patient first began the activity.

When documented, Working Environment start dates will be displayed in the CCDA.

Documents tab > AI Scribe

We’re making updates to improve how providers and support staff collaborate on AI Scribe–generated clinical notes:

  • AI Macros behavior: AI Scribe macros in the patient’s Documents tab will no longer auto-refresh when a saved or draft note is opened by anyone other than the dictating provider. Human scribes and other users can still pull in the latest AI data by selecting Other Actions > Refresh Macros, which updates the note with any additional data from your preferred AI vendor.

  • Support staff access: Once a dictating provider has opened, synced, and saved (or saved as draft) a note, designated non-provider staff will now be able to access, manage, and edit unsigned clinical notes.

Billing

The issue that prevented patients with extensive billing documentation for a visit from transmitting to Ontada Health has been resolved.
